It has been empirically believed that the degree of wall hypertrophy and obstruction of left ventricular outflow tract are the main determinants of cardiac and pulmonary dysfunction in patients with hypertrophic cardiomyopathy, but clinicians often find the contradiction between anatomy and symptoms.

The study published by qiaoshubin, sunxingguo and huxiaoying of Fuwai Hospital said that the labor dyspnea symptoms of patients with hypertrophic cardiomyopathy were related to left atrium enlargement and left ventricular outflow tract obstruction.
Of the 129 patients with hypertrophic cardiomyopathy, 62.8% had labor dyspnea symptoms, 27 (20.9%), 26 (20.2%) and 18 (14.0%) with syncope history.
The results showed that chest pain was independent of coronary heart disease and smoking, palpitation was independent of AF or atrial flutter and decreased heart rate reserve, syncope was independent of the decrease of peak exercise blood pressure.
The researchers have warned that clinicians can choose more targeted and individualized examinations and treatments based on the symptoms of patients with hypertrophic cardiomyopathy.
For patients with hypertrophic cardiomyopathy with chest pain symptoms, it is suggested to improve the examination of hypertrophic cardiomyopathy or coronary angiography, and exclude the possibility of coronary heart disease.
Patients with hypertrophic cardiomyopathy with palpitation symptoms should be regularly checked by electrocardiograph and Holter. Anticoagulation should be performed after AF or AF, so as to prevent thromboembolism.
The study also found that% HRR of patients with hypertrophic cardiomyopathy with palpitation symptoms decreased. The researchers pointed out that this suggests that the patients with heart chronotropic function are more damaged and the cardiopulmonary function is worse.
